Discussed cooling potentially hazardous foods down to 45 F in the Kitchen reach-in cooler before service. 15A NCAC 18A .2804 FOOD SUPPLIES-In Infant Classroom #1, noted some of the infant formula bottles were missing dates on the labels. Observed the teacher attempt to date noted bottles during the inspection. All human milk, formula, and other bottled beverages, including beverages in sippy cups, that are sent from home shall be fully prepared and labeled with the date received at the child care center and the name of the child to whom the milk, formula, or beverage belongs before being brought to the child care center. 5.0
16. 15A NCAC 18A .2815 WATER SUPPLY- The hot water for the classroom hand washing sinks in the "Threes" and "Infant #2" are reading between 113-114 F. For handwash lavatories used exclusively by school-age children, the requirement to provide water tempered between 80 degrees Fahrenheit and 110 degrees Fahrenheit shall not apply. Childcare Director placed a work order to adjust the hot water for noted handwashing sinks during the inspection. 4.0
20. 15A NCAC 18A .2816 LEAD POISONING HAZARDS IN CHILD CARE CENTERS-Noted Facility`s last Lead Water testing was conducted on 11-4-2020. Have plumber lower the water temps to adjust to 80-110 degrees. Plumber came to the facility and corrected all water temps to the 80-110 degrees range. 4.0
26. In all rooms the disinfectant was too weak. Disinfectant shall be held at 500-800 ppm. The testing kit available at the center did not match the colors on the bottle for the kit. All disinfectant bottles were corrected to 500-800 ppm for bleach. Owner will order new test strips. 2.0

37. the paint is peeling off the wall and falling on the floor in the 2 yr. old room behind the area where cots are stored. Walls shall be easily cleanable. Repair this wall. 4.0

observed children in the 4-5 yr. old classroom washing hands. The children did a fantastic job of following the hand wash procedure.

This facility is doing a great job of keeping hazardous products locked and non hazardous products out of reach.

He was able to adjust the tempering valve at the hot water heater and after that all temps in these two classrooms and bathrooms were within the range of 80-110 degrees. Keep ice pack sleeves on these cartons during lunch service. 5.0
10. Feeding trays in the infant rooms are currently being sanitized in the classrooms. Educated the staff that these trays now need to be sanitized in the kitchen since these trays are food contact surfaces. 0.0

16. In the NC Pre K room #2 one hand wash sink was 120 degrees. Maintenance person corrected this to 109 degrees during inspection. All other classrooms had hand wash sinks of 96-109 degrees. 4.0
29. one battery seen in an unlocked drawer in the infant room 1, one container of spritz spray seen in an unlocked drawer in infant room 2, batteries seen on the shelf not locked in the 3 yr. old room. All of these items were moved to a locked drawer or cabinet. Hazardous products shall be stored in a locked cabinet and unlocked with a combination, electronic or magnetic device or key. 6.0
30. one bottle of lotion was seen in a drawer in the infant 2 room but was not stored 5 ft. up. In the toddler room fish food and a purse on a shelf was not stored 5 ft. up. In the 2 yr old room there was a pocketbook not stored 5 ft. up. One bottle of disinfectant in the infant room 2 was not stored 5 ft. up. All of these items were moved to a shelf that was 5 ft. up out of reach. 3.0

11. Throughout the facility, the sanitizing solution was too strong. Proper concentration for disinfectant is 50-200 ppm. Make sure to follow the mixing instructions for the concentration of bleach used and always test solutions after mixing. New sanitizer was made during inspection. 2.0
16. At start of inspection, in the infant, toddler, and NC PreK 1 rooms, the hot water was adequate but in the rest of the classrooms, the hot water was not hot enough. It was at 60-77F. Hot water in areas accessible to children must be maintained between 80-110F. There appears to only be one hot water heater with a mixing valve for the facility. The kitchen prep and dish sinks were reaching 140F but the handsinks were only 70F. The valve was adjusted and the temperatures were corrected in some rooms but then were reaching over 110F in others. Have the water heater, circulating pump, and tempering valves checked and serviced or repaired if needed. Monitor water temperatures to make sure require temperatures are met but not exceeded. 4.0
21. Replace the worn middle toilet seat in the 3s/4s/5s bathroom. More frequent cleaning is needed on the handsinks in the toddlers, PreK, 2s, 3s, and 4s rooms. 4.0
23. Comment: In the infant 2 room, there were toys being kept on the counter by the diaper changing handsink, do not store toys or any other items on the counters by handsinks. 0.0

16. The hot water in all classrooms and bathrooms exceeded 110F. The hot water in areas accessible to children must be maintained between 80F and 110F. The water was turned down during inspection and was between 82F and 108F by end of inspection. Be sure to monitor water temperatures. 4.0
17. Comment: The handsink in the 4s classroom was 120F at start of inspection, the hot water to the sink was turned off immediately. The hot water heater was turned down and the water at the sink was down to 84F by the end of the inspection. Make sure to monitor water temperatures on a regular basis. 0.0
26. The chlorine disinfectant in all classrooms was too weak according to test strips, it was measuring between 50-200 ppm but should have been 500-800 ppm. The high concentration test strips had gotten wet and/or had bleach residue on them and they were not being used. Replace test strips. The disinfectant was changed out during inspection. 2.0
37. Cleaning is needed under the area rugs in almost all classrooms, make sure to lift the rugs and clean regularly. Clean the area rugs in the 3s room. Clean the floors in the 2s storage closet. 4.0
General Comments
In the waddlers room, there is only 1 handsink other than the bathroom sink and the bathroom sink is separated from the classroom by a baby gate. The same sink may be used for handwashing after diaper changing and handwashing before feeding but the handsink cannot be used for a food prep handwashing sink. According to the director, the bottles are not warmed before being given to the children, so as long as proper handwashing techniques are used, it is fine to use the diaper changing handsink for washing hands before feeding. It is recommended to spray the sinks down with disinfectant after washing hands after diaper changing, but not required.
